You'll need:
• A signed application form
• Emirates ID
• Employee ID
• Company stamped salary certificate or stamped pay slip
• Minimum monthly salary of AED 20,000 for the AED 1,200 plan and higher
• Credit-/-debit card copy for auto-payment (not required for UAE nationals)
